The main objective of the project is to develop an integrated care service for people with dementia in the Northern Periphery that supports ageing in place and more cost-efficient care of people with dementia. One under-pinning assumption is that if a template of care works well for the challenging group of people with dementia, then it can easily be transferred and adapted to suit other groups of eldercare receivers. Another assumption is that the core concept of care of people with dementia can be transferred and adapted to the different local contexts of the regions of the Northern Periphery. The template of integrated care services will takes into account state of the art in dementia care and the particular challenges in rural areas. Among these challenges are how to support social and family contact when people are far apart, and how to maintain ageing-in-place for people with dementia. Emerging innovative integrated ICT solutions for persons with dementia will be adapted and their impact will be assessed, to gain the necessary knowledge, stimulate introduction and use of these cost-effective technologies to optimize care of people with dementia in rural areas. The intention is that developed templates of care and concepts of services can be transferred to other groups of elderly people.
